Hours:
Our office is open Monday thru Friday from 8 am until 5 pm. We take lunch daily
from 12-1. All non-urgent questions regarding your care should be directed to the
office during these hours. After hours, Dr. Gayoso is easily accessible by cell
phone for messages that can not wait until the office reopens.

Appointments:
We make every effort to see patients at their scheduled time. In order to be successful
in this effort, we ask that you arrive for your appointment on time and prepared to see
the doctor. Dr. Gayoso takes great pride in spending quality time with each patient. If
you are unable to keep your appointment, kindly give us as much notice as possible so
that we may utilize the time effectively for other patients.

Gift Certificates:
We offer Gift Certificates in any denomination amount. This is a convenient and
thoughtful way of introducing loved ones and friends to Natural Image's various services.
Dr. Gayoso will ultimately determine which surgical or non-surgical treatment would be
most appropriate for the prospective or current patient.

Insurance:
We participate with many major insurance plans, some of which may require a formal referral
in order for you to see Dr. Gayoso. If you are unsure if your insurance plan requires a
referral, we ask that you contact them prior to your office appointment to obtain this
information. This referral is important in order to avoid a possible insurance denial of your
visit. If you present to your appointment without a referral, we will need to reschedule your
appointment once a referral has been obtained.

Financing:
Payment for services is expected at the time of your appointment. We accept cash, checks, Visa
and MasterCard. As a courtesy, we will bill your insurance company but you will remain
responsible for any procedures/fees not covered by your insurance.
Financing is available for those patients undergoing elective cosmetic surgery. We offer financing
services thru Capital One and Care Credit and will provide you with this information upon your request.

Confidentiality/Privacy:
Patient confidentiality is a priority at Natural Image Plastic Surery. We will not release any information
regarding you or your surgical care without your written consent. Should you wish to allow family members or
friends to have access to information regarding your care, please let us know so that we may complete the
necessary paper work.
